We offer online preregistration through MyChart for many inpatient and outpatient services at UAMS.
If you use online preregistration, you will need to preregister at least five days before your scheduled service. You can preregister for surgery, a test or procedure if it has already been scheduled or if admission to the hospital is planned.
Otherwise, you can preregister by telephone until the day of your visit. Please call 501-686-6236 between 9:00 a.m. – 6:00 p.m., Monday – Friday.
Be prepared to provide your:
- Date of surgery, test or procedure
- Demographic information (age, sex, address, etc.)
- Insurance information (see your insurance card)
- Doctor’s name(s)

Documents Needed for Registration
The UAMS Medical Center Registration area is located on the first floor of Ward Tower. After you arrive, a UAMS Registration representative will meet with you and your family to finalize your admission and get the information necessary for your hospital stay.
Please have the following items available if you have them:
- Insurance Card(s)
- Doctor’s Orders (if available)
- Medicare Card/Medicaid Card
- Drivers License or photo ID
- Social Security Number
- Authorization Document (HMO/PPO patients)
- Pre-certification Document
- Referral Form
Insurance Details
Many PPO, HMO, employer benefit programs and insurance plans require pre-certification for payment of full benefits. If your insurance policy requires pre-certification or a second opinion for treatment, you need to provide appropriate authorization numbers and documentation before care is given. Also, if your plan requires a referral form, please bring it with you.
Insurance information must be provided within 24 hours of admission in order to meet our requirements for timely notification to the payor. It is your responsibility to work with your physician and insurance carrier to meet the requirements of your policy. Failure to do so may result in a denial or reduction of insurance coverage.
Patients Without Insurance Coverage
Patients without insurance coverage will be expected to make a deposit on admission. A financial counselor will assist you in making arrangements. Programs are available for qualified applicants who are unable to meet their financial obligations. UAMS Medical Center staff can facilitate applications for Medicaid or other assistance programs.
