About the Health Information Management Department
The Health Information Management Department at UAMS Health fulfills requests for release of medical records for all UAMS Health patients.
Our staff will be happy to assist you in obtaining copies of medical records for UAMS Health patients. You can fax, mail, email or personally deliver your request for records.
Since UAMS Health procedures ensure that protected health information remains private, we will need a valid authorization with all third-party requests. Valid authorization forms are located on the UAMS HIPAA site.
There is no charge for obtaining copies of a patient’s medical records if the records are sent to a doctor’s office, clinic or hospital.

Electronic Health Records
This computer system lets UAMS Health providers directly involved in your care quickly find, review and update your record in all the clinics and in the hospital.
