Appointment Process

We strive to make the appointment process smooth and stress-free. We have provided an outline of what to expect from start to finish.

Step 1: Intake

Referral

Your child’s provider sends a referral to the Dennis Developmental Center to start the Intake Process. Please note that the processing time for all referrals is 24-72 hours.

PCP Referral Form 

PIP (Parent Intake Packet)

Upon receipt of a PCP referral from your child’s doctor, a PIP will be mailed to you to complete. Once the PIP has been mailed, our Family Navigators will contact you to discuss pre-triage recommendations and sign you up for MyChart access. You may also download this packet and fill it out.

Completion

Complete the Packet and return it to the James L. Dennis Developmental Center, which moves the process to Step 2, Triage.

Step 2: Triage

Decision

The Dennis Developmental Center decides if your child’s needs are appropriate for the Dennis Developmental Center. If your child’s needs fall within the scope of services offered by the Dennis Developmental Center, an initial appointment is scheduled based on the triage decision.

Our Family Navigators will follow up with you to discuss the triage decision, additional recommendations and wait times.

Step 3: Waiting for Appointment

Family Responsibilities

Families can use this time to gather prior testing, past evaluations, and therapy evaluations for Occupation, Speech and/or Physical therapies. Providing these documents before the appointment is helpful for our clinicians to have a better understanding of your child’s needs.

Step 4: Appointment Scheduled

Once there is an appointment slot available, you will be contacted by our clinic schedulers. When the appointment is scheduled, we will send you an appointment reminder, form for your child’s school/daycare provider to fill out and information about what to expect at your child’s appointment.

Please note that active insurance is required to schedule an appointment. If your insurance coverage changes, you lose your insurance, or you plan to self-pay, please contact us as soon as possible at 501-364-1830 so we may assist you.
