The University of Arkansas for Medical Sciences is mandating COVID-19 vaccination for all its employees, non-employees and students. Each request for exemption, regardless of the reason, is evaluated individually by Student and Employee Health Services, and if needed, the Office of Human Resources.
Non-employees are defined as non-UAMS workers, contractors, visiting students, visiting faculty, pharmaceutical representatives, traveling nurses, locum tenens physicians, custodians, and employees of businesses who work on a UAMS campus.
Exemption Forms
All employees wishing to obtain either a religious or medical exemption from the COVID-19 vaccine need to complete and sign the first page.
Medical Exemptions
Anyone seeking a medical exemption needs to have the medical exemption form completed by their physician or medical provider and emailed it to UAMS Student and Employee Health Services.
Religious Exemptions
Employees seeking a religious exemption are required to complete and return the religious exemption form and email it to UAMS Student and Employee Health Services.
