The UAMS El Dorado Family Medicine Center provides primary care medical services to patients of all ages. We cover all stages of life from pediatric conditions to senior health care.

The clinic provides convenient diagnostic and support services on site. We are also able to perform minor office surgical procedures in the clinic. We offer routine adult and pediatric immunizations

Prescription Information

To refill a prescription, please ask your pharmacy to send an eScribe request to our clinic, or you can request a refill during your appointment. If you originally filled your prescription at the UAMS Health Outpatient Pharmacy, you can also refill your prescription online using MyChart.

What to Bring to Your Appointment

Their photo ID, their insurance card, a list of any medications they’re taking, any medical records they can provide (including slides or test results from other clinics)

What to Expect at Your Appointment

For your first appointment, please check in 15 minutes before your appointment time. We will need to make sure we have your correct contact information, date of birth, address, a working phone number, and email, if you have one. We will need to verify your insurance. All co-payments and deductibles are due when you check in for your appointment.

Your first visit will be your longest visit, as we will need to enter your information into our Electronic Medical Record. This visit will include a physical exam and we will go over your medical history. Please bring with you all medicines that you take (in their original bottles), including prescriptions, over the counter, vitamins, and herbal supplements.
