Medication Assistance Program (MAP)

The UAMS Outpatient Pharmacy Medication Assistance Program (MAP) is designed to help UAMS patients access medications prescribed by their UAMS providers. Our goals is to improve patients’ health and quality of life by helping them pay for their medications.

Examples of Medications that are NOT covered

Controlled Substances:

  • Oxycodone
  • Hydromorphone
  • Fentanyl
  • Morphine
  • Methadone
  • Codeine
  • Z-Drugs (Insomnia)
  • Sudafed
  • Anorectics (weight loss)

Over-the-Counter Drugs (OTC):

  • Loratadine, Cetirizine, Fexofenadine
  • Probiotics
  • Vitamins/Supplements
  • Tylenol, Ibuprofen, Naproxen
  • Aspirin
  • Meclizine, Dramamine
  • Artificial Tears, Ketotifen, Pataday
  • Benadryl
  • Hydrocortisone Cream
  • Laxatives

Please Note:

  • Not all prescription medications are covered on the program.
  • Over-the-counter and non-prescription medications are NOT covered
  • Controlled medications (opioids, benzodiazepines) are NOT
  • Prescriptions must be written by a UAMS provider.
  • Medications covered are subject to change without notice.

Qualified applicants will need to complete the MAP application. If you are non-medicaid recipient, you will also need to provide the required supporting documents.

Qualifications for Assistance

  • Patients must be seen at a UAMS clinic by a UAMS provider.
  • Prescriptions must be written by a UAMS provider.
  • Patients must meet financial qualifications.
    • Annual family gross income (before taxes and deductions) must be equal to or less than 300% of the current Federal Poverty Level (FPL). You may check current FPL here:
    • Patients out of pocket (OOP) prescription spend for the current calendar year totals 6% or more of the total family gross income.
  • Patients with no prescription coverage/insurance may qualify.
  • Patients with active Arkansas Medicaid requiring more than six monthly medications or with medications not covered by Arkansas Medicaid may qualify.

Required Documents

Non-Medicaid recipients are required to provide the following documentation:

Proof of Household Income with at least one of the following:

  • Social Security award letter for the current Calendar year
  • Unemployment Letter
  • Current W-2 or 1099 Forms
  • Last three consecutive pay stubs
  • Pension statement
  • Last three consecutive bank statements

Proof of Legal Residence with at least one of the following:

  • Driver’s License or government issued photo ID
  • Social Security Card or Green Card

Active Medicaid recipients are required to provide only the MAP application upon UAMS verification.

Helpful Tips

Apply for Arkansas Medicaid before calling the MAP office by calling the UAMS Medicaid office at 501-686-7680. Call 800-772-1213 to get a current social security benefit letter. If you can’t find your W-2, you can get a tax transcript online at https://www.irs.gov/individuals/get-transcript.

How to submit your MAP Application:

MAP applications and supporting documents may be submitted in person, by email, mail or fax. In-person and phone appointments are available to make sure you have all the documents you need for your application.

Telephone and in-person appointments are available between 9:00 a.m. and 4:00 p.m. Monday through Friday. If you need to cancel your appointment, please call 501-686-8953 to cancel at least 24 hours in advance.

Directions to MAP Office: Park in Parking 3 at the corner of Capital and Cedar streets. Follow signs to the Outpatient Center. The MAP office in the Outpatient Pharmacy on the first floor. If you are using GPS driving directions, use the address 4000 West Capital Avenue 72205 for Parking 3.

Important Information

Patients must be reapproved once a year (12 months from last enrollment) with updated documents. To prevent delays, be sure to renew your enrollment before the current enrollment expires. Patients may be dismissed from the MAP program at any time for failure to comply with MAP rules.
