- Tylenol, Ibuprofen, Naproxen
- Aspirin
- Meclizine, Dramamine
- Artificial Tears, Ketotifen, Pataday
- Benadryl
- Hydrocortisone Cream
- Laxatives

- Not all prescription medications are covered on the program.
- Over-the-counter and non-prescription medications are NOT covered
- Controlled medications (opioids, benzodiazepines) are NOT
- Prescriptions must be written by a UAMS provider.

Qualifications for Assistance
- Patients must be seen at a UAMS clinic by a UAMS provider.
- Prescriptions must be written by a UAMS provider.
- Patients must meet financial qualifications.
- Annual family gross income (before taxes and deductions) must be equal to or less than 300% of the current Federal Poverty Level (FPL). You may check current FPL here:
- Patients out of pocket (OOP) prescription spend for the current calendar year totals 6% or more of the total family gross income.
- Patients with no prescription coverage/insurance may qualify.
- Patients with active Arkansas Medicaid requiring more than six monthly medications or with medications not covered by Arkansas Medicaid may qualify.
Required Documents
Non-Medicaid recipients are required to provide the following documentation:
Proof of Household Income with at least one of the following:
- Social Security award letter for the current Calendar year
- Unemployment Letter
- Current W-2 or 1099 Forms
- Last three consecutive pay stubs
- Pension statement
- Last three consecutive bank statements
Proof of Legal Residence with at least one of the following:
- Driver’s License or government issued photo ID
- Social Security Card or Green Card
Active Medicaid recipients are required to provide only the MAP application upon UAMS verification.
How to submit your MAP Application:
MAP applications and supporting documents may be submitted in person, by email, mail or fax. In-person and phone appointments are available to make sure you have all the documents you need for your application.

Telephone and in-person appointments are available between 9:00 a.m. and 4:00 p.m. Monday through Friday. If you need to cancel your appointment, please call 501-686-8953 to cancel at least 24 hours in advance.

Important Information
Patients must be reapproved once a year (12 months from last enrollment) with updated documents. To prevent delays, be sure to renew your enrollment before the current enrollment expires. Patients may be dismissed from the MAP program at any time for failure to comply with MAP rules.
