What should I bring to the appointment?

For your first appointment, please bring a picture ID and your insurance card. The UAMS medical staff will also need information about any medications you’re taking, including over-the-counter medications. You should also bring any medical records you can provide, including x-rays and test results from other clinics.

Physical therapy patients need to bring attire and shoes appropriate for exercising.

For future appointments, please provide copies of any additional x-rays or test results, and advise us of any changes in your insurance information or your medications.

How do I request a prescription refill or contact a nurse from the clinic?

Call the clinic at 501-686-6102, or you can request prescription refills during your appointment. If you originally filled your prescription at the UAMS Outpatient Pharmacy, you can also refill your prescription online.
