Appointments in the Physical Medicine and Rehabilitation Clinic may require a referral, based on your insurance requirements. Please contact your primary care doctor or visit one of our doctors in the Center for Primary Care to obtain a referral.
If you prefer to call and request an appointment, please call 501-686-8000 for new patients, returning patients and referral appointments.
What should I bring to the appointment?
For your first appointment, please bring a picture ID and your insurance card. The UAMS medical staff will also need information about any medications you’re taking, including over the counter medications. You should also bring any medical records you can provide. Also, bring a completed copy of the Patient Health Questionnaire sent to you after you set your first appointment.
For future appointments, please advise us of any changes in your insurance information or your medications. This clinic also requires that all new patients pay for their first visit at the appointment
How do I request a prescription refill or contact a nurse from the clinic?
Call the clinic at 501-686-5838 to request a prescription refill, or you can request prescription refills during your appointment. Your pharmacy can fax a prescription refill request to the Clinic at 526-7467. You may need to provide your pharmacy’s name and contact information during the phone call.
If you originally filled your prescription at the UAMS Outpatient Pharmacy, you can also refill your prescription online. If you need to contact a nurse in the clinic, call the clinic at 501-686-5838. You may need to provide your doctor’s name to speak with the correct nurse.
