UAMS House Call Program
In today’s hurry-up world, doctors making house calls seems to be a thing of the past. But the UAMS Integrated Medicine medical team knows it can be a lifesaver. Our goal is to provide medical care and care coordination that allows frail elderly and other chronically ill adult patients to maintain health and quality of life while remaining in the home.
Preliminary research indicates that a home medical visit may reduce the inpatient length of stay and decrease the rate of readmission to the hospital for frail, chronically ill, and other homebound patients. These services are also provided to patients in assisted living facilities.
The House Calls Program is designed to extend the continuum of care for those patients who are unable to visit the doctor’s office. The House Calls physicians and Advanced Practice Nurses (APRN) can be your primary care team or work with your existing doctor to provide care during transitions from the hospital, emergency room, or skilled nursing facility back to home. The House Calls doctors, in collaboration with an APRN, can coordinate care between families, nursing agencies and home health services. Our team also includes nursing, medical assistant, and social work staff to assist in your care.
The House Calls Program serves the adult population within a 30-mile radius of UAMS for primary care and transitional care services.
All routine medical care includes:
- Physical exams
- Regularly scheduled follow-up visits
- Acute visits for other sudden illness
- Immunizations such as flu and pneumonia shots
- Prescription refills, lab work, x-rays
- Referral to appropriate home health services, durable medical equipment, rehabilitation services, social service & community resources information and linkage
Transitional care services are provided following discharge from the hospital, emergency room, and skilled nursing facilities.
Medicare, Medicaid/dually eligible, private insurance or private pay insurance is accepted, and we take care of all the hassles of billing.
