Although UAMS does not require a physician referral for some services in this clinic, some insurance providers require a referral for specialty care. Please check with your insurance provider before requesting an appointment.
What should I bring to the appointment?
For your first appointment, please bring a photo ID and your insurance card along with a list of any medications you’re taking, including over-the-counter medications. You should also bring any medical records you can provide, including X-rays related to your visit and slides or test results from other clinics.
For future appointments, please provide copies of X-rays and any additional updated medical and insurance information.

Location: 1st floor of the Outpatient Center building
Parking: Parking is available in Parking 3 at the corner of Cedar and Capitol. Valet parking is also available in front of the Outpatient Center building.
Go to level 3A in the parking deck and take the bridge to the 1st floor of the Outpatient Center Building.
