Appointments with the Audiology Clinic are by referral only. Please contact your primary care doctor or visit one of our doctors in the Center for Primary Care to obtain a referral.
If you prefer to call and request an appointment, please call 501-686-8000 for new patients and referral appointments or 501-686-5940 for returning patients.
What should I bring to the appointment?
For your first appointment, please bring a picture ID and your insurance card. The UAMS medical staff will also need information about any medications you’re taking, including over-the-counter medications. You should also bring any medical records you can provide, including any hearing evaluations from other clinics, as well as any current hearing amplification devices.
For future appointments, please advise us of any changes in your insurance information or your medications.
How do I request a prescription refill or contact a nurse from the clinic?
Call the clinic at 501-686-5940, or you can request prescription refills during your appointment. If you originally filled your prescription at the UAMS Outpatient Pharmacy, you can also refill your prescription online.
If you are an ear, nose and throat (ENT) patient and need to speak to a nurse, call 501-686-5878. You may need to provide your doctor’s name to reach the correct nurse.
